Medical Weight Loss: Weight Check

Service Description

Members of the program get 1 free monthly weight check with the provider, but can have unlimited access to the basic scale in the wellness center during business hours. If using the scale more than once per month, participants are responsible for recording their weight.

Cancellation Policy

In order to best serve our patients and well as compensate our staff, we require the following levels of notice in advance of your appointment for cancellations and/or rescheduling: - Individual- 24 Hours - Parties- 5 Days - Events- 10 Days Failure to provide the appropriate notice (listed above) will result in the following fees: - Individual- $50 - Parties- $50 per no show or late cancellation, or 50% of the price of the goods or services requested (whatever is greatest). - Events- 50% of the price of goods and services requested for the timeframe requested.
